Description :

The Intake Coordinator is responsible for assisting with the coordination of admissions activities; provides secretarial services and other clerical functions by performing assigned duties. Demonstrate a positive and professional attitude and working relationship with Northern Illinois Hospice staff and all points of contact.

Essential Duties and Responsibilities :

  • Responds to incoming admission calls in a timely and professional manner.
  • Initiates intake / referral information.
  • Enters patient referrals into the electronic medical record and assists with the tracking of pending referrals.
  • Helps educate potential hospice patients, families, and referral sources.
  • Provides statistical summary of activities and other data for the referral / admission process.
  • Serves as an active collaborative member of the Admissions & Education Team providing information on referral / admissions sources.
  • Provides clerical support to the Admissions Staff.
  • Assures medical records are scanned into electronic medical record in a timely manner for admissions and pending patients.
  • Completes database records for referral patients not admitted and routes referral lists to Director.
  • Ensures medical record audits are completed for all admissions in a timely manner.

  • Ability to work in a fast paced, emotional environment and maintain positive work attitude while working with all points of contact.
  • Ability to maintain professional presence at all times including personal appearance and communication with all points of contact.
  • Maintains a working knowledge of Medicare, Medicaid, and Insurance provider requirements for Hospice eligibility and completes Medicaid and private insurance pre-authorizations as required.
  • Education : High School graduate (or equivalent) with some college or technical school training; Associate degree preferred.

    Knowledge / Skills / Ability : Medical assistant experience preferred. Excellent verbal and written communication skills.

    Experience : Minimum 2 years of hospice and / or healthcare clerical experience required

    Other : Meets agency requirements for criminal background check and drug screen.

    Expected Hours of Work : Monday – Friday 8 : 15a -4 : 45p
