Job Overview
This position is responsible for coordinating and scheduling the individual resident assessments in accordance with facility policy, state and federal regulations. Assist the Director of Nursing, families, residents, and staff nurses with the coordination and management of individual resident care needs. This position exercises discretion and independent judgment in regards to matters of significance. The person within this role will also assist with educational needs for team members at Northern Lights.
Duties
- Acquires and maintains current education of new MDS item sets, NTAs, comorbidities for accurate coding, and reimbursement
- Tracks and schedules required RAIs per state and federal requirements.
- Determines and schedules appropriate MDS assessment dates and communicates to the appropriate team members.
- Coordinates, develops and maintains current resident assessments as required including initial, quarterly, annual, and all OBRA required MDSs to assure resident/nursing care plans and all supporting documentation is present.
- Responsible for the coordination, encoding, and electronic submission of Medicare/Medicaid/OBRS/mandated MDS/RUGS/and other related documents as federal and state required.
- Monitors nursing notes to assure they are clear, concise, informative and descriptive of the nursing care provided.
- Arranges assessments accurately to capture highest possible RUG/CMI score.
- Coordinates reviews and notifies resident appropriateness of Medicare, or other coverage per policy/procedure/regulation.
- Assists in the coordination and ongoing education for the staff in understanding ADL documentation.
- Maintains an up to date Roster Matrix.
- Assist DON in other duties as assigned.
- Performs other duties as assigned.
Requirements
- Previous experience in a hospital or clinic setting (strongly preferred)
- Proficiency in MDS assessment tools and software (strongly preferred)
- Knowledge of Medicare regulations and clinical documentation standards (strongly preferred)
- Strong attention to detail and organizational skills
- Ability to prioritize patient care needs
