Northern Montana Hospital (NMH) is the center of a comprehensive system of medical and health care services, staffed with a team of caring professionals who are dedicated to meeting the growing needs of our Hi-Line communities. The not-for-profit, 25-bed Critical Access Hospital features a 24-hour physician-staffed emergency department, with state-of-the-art diagnostic imaging and electronic medical record services. Our large, quiet labor/birthing rooms are private and located away from other patient care areas. The private patient rooms on the 5th floor Medical/Surgical Department boast views from every window.

Administration, staff, and medical providers are committed to delivering high quality, comprehensive health care services to the people of the Hi-Line communities of Montana.

Specialties of our active staff of doctors include Anesthesiology, Emergency Medicine, Family Medicine, General Surgery, Hematology, Internal Medicine, OB/GYN, Oncology, Ophthalmology, Optometry, Orthopedic Surgery and Sports Medicine, Pain Management, Pathology, Podiatry, Psychiatry, Psychology, Radiology and Women’s Health.

For the convenience of our patients, we also host visiting physicians at our clinics in the areas of:

About the role

The Chronic Care Coordinator’s primary responsibility is to assist in the overall management of Northern Montana Hospital (NMH) patients, particularly those with chronic disease. Demonstrates innovation and an acceptance of new ideas and concepts. Shows the ability to resolve conflicts with clients and overcome their objections about their health and behaviors. Possesses above average optimism and the ability to motivate clients to make vital changes in their lives. Exhibits effective communication skills for working with clients, their families and their other healthcare providers. Demonstrates a commitment to clients and a willingness to go above and beyond normal responsibilities to provide the best care possible. Shows self-confidence and the ability to express views that may not be popular or shared by clients. Performs population health, care management, and patient-self management support and clinical documentation functions. Provides support for patient and family centered continuity of care services for patients undergoing transition of care. Assists with providing patient and families with optimal linkage to community resources. Works collaboratively to improve quality of patient care through facilitating the efficient use of resources. Acts as an advocate for individual’s health care needs and assists in minimizing the fragmentation of health delivery systems.


Qualifications

  • Degree in health education, nursing, nutrition, or certification as Registered Medical Assistant or three (3) years clinical experience in a primary care setting, or equivalent education or experience.
  • Will have up to date training and certification in Healthcare Provider Basic Life Support issued by the American Heart Association, American Red Cross or American Health & Safety Institute. Must obtain within three (3) months if not current at hire.
  • Able to obtain certification through the National Society of Health Coaches. Salary increase upon certification.
  • Valid driver’s license with acceptable driving record for insurance purposes
  • Ability to successfully pass background check and drug screen.

Salary is commensurate with up to 15 years of experience.
