What are the responsibilities and job description for the Case Manager- BH position at Northern Nevada HOPES?
Case Management services serve as a hub for communication, collaboration, and support for patients working with interdisciplinary teams. Case Managers at HOPES provide Non-Medical Case Management services to patients by completing program screening and providing referrals and related service navigation that improve patient overall health outcomes. Intake services include identifying patient’s needs and provide linkage to services at Northern Nevada HOPES and coordinate with community partners.

- Behavioral Health Case Management - Medicated Assisted Treatment (MAT), Intensive Outpatient (IOP), Outpatient Behavioral Health. Behavioral Health Case Management (BH CM) includes providing educational services to increase program participant’s skills in accessing resources for continued health behaviors. BH CM services are provided for patients enrolled in HOPES Behavioral Health Programs and services. BH Case Managers participate as integral members of the interdisciplinary treatment team in the Intensive Outpatient BH Treatment Programs, the Medication Assisted Treatment Program.
