What are the responsibilities and job description for the Addiction Counselor position at Northern Path Recovery Center ?
Must have 1 of the following approved by the DMHA.
Licensed Clinical Addiction Counselor (LCAC) Licensed Addiction Counselor (LAC) CADAC II ADAC III CADAC IV CADAC V Addiction Professional In Training (APIT) Certified Addiction Counselor Level I (ICACI) Certified Addiction Counselor Level II (ICACII) NCAC I NCAC II Master Addiction Counselor (MAC) Participates in patient treatment planning and continuum of care.
Conducts group and / or family sessions, under supervision, as scheduled and documents as required. Co-facilitates assigned group or family sessions and assists with modifying problem behaviors. Responsible for initial assessments, as well as follow up assessments for patients. Prepares individual treatment plan for each assigned patient. Ensures all documentation regarding patient care, treatment and incidents is completed timely and in a clear, concise manner. Acts as a liaison between referral sources and patients. Provides crisis intervention to patients, as needed. Plans for aftercare for assigned patients. Acts as a liaison with insurance carriers, as requested. Provides case management duties for patients, ensuring individualized quality care. Evaluates the need of the patient and determines if referrals to other programs or facilities is needed. Provides patient and family education relative to substance abuse / psychiatric problems. Conducts educational workshops both within the Hospital and in the community as required. Serves as a liaison with self-help groups and EAP professionals. Conducts ongoing assessments of chemical dependence services required by the Hospital treatment programs. Requires Bachelor's degree in social or health services field, with 2 years exp in counseling field; OR Bachelor's degree in other field with 4 years in the counseling field; OR Master's degree in social or health services field, with 1 year professional work experience. Degree must be from an accredited college or university. Licensure or certification for counseling may be required by the state in which the facility operates. CPR and de-escalation certification required (training available upon hire and offered by facility). First aid may be required based on state or facility.
