The NY Connects/ ICAN Advocate is responsible for assisting people with questions and concerns with long term care and assisting people insured by Medicaid and need help with long term care or behavioral health services. The goal of NY Connects is to provide information and assistance to all individuals seeking support with a focus on supporting the participant’s independence and self-determination. The goal of ICAN is to assist people to enroll in and use managed care plans that cover long term care or behavioral health services so they can maximize their independence.
ESSENTIAL JOB DUTIES:
1. Provide private one-on-one counselling about options and resources by telephone or in-person appointments.
2. Answer questions related to Medicare, Medicaid or long term care
3. Build community partnerships with the local DSS and OFA office and other community-based organizations to provide assistance to participants
4. Project staff will be fully prepared to
a. administer the No Wrong Door (NWD) screening tool and online applications
b. utilize Person-Centered Assistance/Options counseling techniques to assist participants
c. coordinate with other NWD partner agencies
5. Promote self-help and self-advocacy strategies.
6. Assist people to learn benefits entitlements such as social security, vocational assistance, social services, housing assistance, adult and child protection services, home health care and personal assistance services, person in need of petitions, Medicaid and foodstamps.
7. Provide individual advocacy and when needed attend hearings, administrative reviews and appeals with individuals.
8. Ensure consumer control of services received.
9. Provide peer counseling when appropriate.
10. Maintain up to date list of community resources and share information with community members and staff.
11. Complete all reporting and record keeping as required.
NONESSENTIAL JOB DUTIES:
1. Represent NRCIL at forums and public meetings in the community.
2. Assist in coordinating workshops and seminars.
3. Collaborate with co-workers to involve consumers in providing direction to NRCIL programs and participating in center advocacy efforts and activities.
4. Assist in identifying community needs and in executing agency goals.
5. Actively promote disability rights and independent living philosophy through all activities conducted within and on behalf of NRCIL.
6. Work outside of regular office hours when necessary.
7. Performs such additional duties as may fall within the scope of said position and ability of employee as requested by Supervisor or Executive Director.
8. Actively cooperate with all NRCIL staff to achieve Center’s mission.
9. Devise a plan for unexpected absence from this position; file plan with support staff.
PREFERRED QUALIFICATIONS: Associate’s degree and minimum 3 years work experience preferred, or appropriate combination of skills, experience, and education. Must have excellent verbal and written communication skills. Must be well-organized, able to work independently and able to meet deadlines. Must have interests in disability rights and familiarity with the concept and philosophy of the independent living movement. Preference will be given to applicants who have personal experience with a disability. EEO.
