Job Summary
The Physical Therapist position directs the assessment, treatment planning and implementation of therapy services to patients of all ages, utilizing current knowledge, theory, evidence and sound clinical judgment. Works closely with the patient, family and other members of an interdisciplinary team to provide efficient and effective individualized patient care. Serve as educators and mentors for therapy students, peers and other medical professionals.
Compensation
We are committed to providing wage ranges for all open positions. Please note that the specific compensation for this role will be determined based on experience, education and internal equity considerations. The hourly wage range for this position is: $40.25-56.82. This range reflects the base salary for this position.
Key Accountabilities

Conducts an appropriate evaluation of the patient and documents accurate information within the medical record.

Develops and implements age-appropriate functional goals and treatment plan based on referral, the patient’s history, examination findings, and patient’s input.

Re-evaluates goals and treatment plan at appropriate intervals and modifies the therapy program based on the patient’s response to treatment to ensure optimal outcome.

Provides timely discharge planning including equipment needs assessment, identification of community resources where needed and completion of home program/discharge summary.

Develops appropriate treatment interventions, considering indications and contraindications, based on patient’s current level of functioning.

Completes accurate and timely documentation of the patient’s progress and the treatment performed that meet professional standards.

Communicates/collaborates with interdisciplinary team members regarding patient progress, response to treatment, goals and program changes.

Clearly presents verbal and written instructions to patients and their families in appropriate terminology.

Pursues continuing education opportunities
Qualifications
-
Licensed by the Minnesota State Board of Physical Therapy to practice in Minnesota

Prefer 2 years of outpatient orthopedic experience

Current American Heart Association BLS for Healthcare Providers Certification (or obtained within six months of hire, company paid)
Schedule
-
64hrs/2wks
-
Monday-Friday, shift start and end times vary between 7:30am and 6:00pm
-
Option for four 8-hour days or some combination working five days a week
-
This position will be scheduled to work 2–3 weekends a year and one holiday every other year
