Assertive Community Treatment Housing Specialist- Summary
NorthKey Community Care is seeking a full-time Assertive Community Treatment (ACT) Housing Specialist to work with adult clients with serious mental illness (SMI) and to provide housing services in order to foster independent living in the community. This position will be based at NorthKey's location in Owenton, Kentucky but may travel to other locations.

Assertive Community Treatment Housing Specialist- Qualifications
- High school diploma or GED required, Bachelor's degree preferred.
- Valid driver’s license and reliable transportation required.
- At least one year experience working with individuals with mental health disorders or co-occurring mental health and substance use disorders and one year experience with housing is preferred.
- Ability to prepare written reports and records.
- Knowledge of federal, state and local housing programs would be helpful.
Assertive Community Treatment Housing Specialist- Responsibilities & Duties
- Develop and maintain a working relationship with area landlords
- Schedule housing interviews
- Assist with rental application process
- Organize community support and conduct outreach for supportive housing activities in service area
- Acquire knowledge of various federal, state and local housing programs
- Develop a working knowledge and resource guide of all local housing options
- Provide individual activities necessary to assist individuals with mental illness or co-occurring mental illness and substance use disorders to live successfully in the community through utilization of skills training as identified in the individual’s treatment plan
- Provide transportation and/or assist client with obtaining transportation to meeting location
- Document all contacts with client in the electronic health record within required timeframe
- Successfully complete the state approved training program within six months of hire
- During each subsequent year of employment, complete and maintain documentation of a minimum of six (6) hours of training or continuing education
