What are the responsibilities and job description for the Bilingual Case Manager position at Northwest Family Services?
This position coordinates the client’s on-going care from intake to discharge to ensure a positive service experience for youth and family.
What’ll you do:
· Conduct pre-engagement and intake meetings with youth and families who have been referred to the program.
· Identify client barriers to services and/or support and then design short-term solutions.
· Refer clients and/or their family members to services and resources as needed.
· Develop and maintain professional relationships with schools, juvenile justice, BHRN partners and other relevant community partners.
· Follow up on client progress after they have been given recommendations by the Medical Director.
· Follow up with clients and referral resources as needed.
· Maintain client documentation and ensure documentation follows Oregon Administrative Rules (OARs) as well as organization standards, protocols, procedures, and timelines.
· Serve as a positive role model for youth and positive support for families.
· Regular attendance Required.
· Other duties as required.
General Responsibilities
· Follow the protocol, policies, and procedures of the department and agency.
· Complete and submit reports as needed.
· Maintain and adhere to all Federal, State and County regulations regarding client confidentiality to include an understanding of 42 CFR-Part 2 and HIPPA.
· Provide culturally responsive services that are equitable, respectful, and relevant to people from a wide variety of cultures, backgrounds and orientations.
· Build rapport with youth clients, family members and community members
· An understanding of trauma informed work as well as those youth and adults who are affected by adverse childhood experiences.
· Attend meetings and trainings as required.
· Proficiency in Microsoft Office products such as Word, Excel, and Outlook
· Regularly participate in professional development.
Who you are:
· A high school diploma or equivalent is required.
· Bilingual in both English and Spanish is required for this position to include the ability to speak, read, and write fluently in English and Spanish.
· Experience with electronic health record system is preferred.

Pay: $22.00 - $24.00 per hour
