Behavioral Health Case Manager
Location: West Salem Clinic – Mental Health
Position Status: Full-Time
Office Hours: Monday - Friday | 8:00 a.m.–5:00 p.m.
YOUR ROLE:
The role of the Behavioral Health (BH) Case Manager is to provide resources, coordination, support, and wrap around services for assigned clients of Northwest Human Services, including the Mental Health, Connections, and Psychiatric Programs. This includes a variety of activities ranging from basic case management and resource development to functioning as the liaison between the patients, community, and Northwest Human Services. The BH Case Manager is a member of the Care Coordination Team, working alongside a multi-disciplinary team of Community Health Workers, Mental Health Providers, and Medical Professionals to address barriers to positive health outcomes.

SPECIFIC DUTIES:
- Build rapport, practice active listening, show empathy and compassion, demonstrate cultural awareness and humility, and be honest and transparent. Manage conflict effectively. Be open about roles, responsibilities, and limits, and follow through on all agreed upon tasks. Build trust and rapport with colleagues and other professionals.
- Utilize established care coordination workflows to offer client-centered, individualized supportive services and coordination of care. Make referrals, manage calls, set treatment plan goals, and facilitate group settings, with the goal of empowering clients to set and work towards their own goals. Interventions will include, but not be limited to, connecting clients to community resources, building self-advocacy skills, determining readiness for change, navigating ambiguity, installing confidence, optimism, and validation. Assist clients in attending medical and mental health appointments, ensuring that they receive the care they need to maintain their health and well-being. Network with community-based organizations and other resources to promote client services, care, education, and advocacy.
- Document all assigned referrals, tasks, and interventions in NextGen. Maintain up-to-date, timely, accurate, and complete care coordination notes within the electronic health record and follow procedure for appropriate follow-up. Be organized and utilize a systematic approach to information management. Collects data and tracks interventions and outcomes to support strategic planning processes. Submit appropriate billing codes.
- Carry a case load and use judgment to determine the frequency, quantity, and timing of follow-up interventions. Assist clients in developing and implementing a care plan tailored to individual needs and concerns, with the goal of improving basic functioning in daily and community living. Provide closed-loop communication back to referral sources as needed.
- Work within an interdisciplinary care team, prioritizing and responding to TEAMS, tasks, referrals, and phone calls. Handle multiple priorities while remaining professional, utilizing strong interpersonal and communication skills and practicing closed-loop communication. Contribute to teamwork, serving as a liaison, client advocate, and coordinator among program staff who work with the client's care, including any Northwest Human Services program staff and/or other essential supports such as medical, community-based housing, corrections, and mental health providers. Please note that all staff members may occasionally need to shift their breaks, lunch, and beginning/end of shift times to meet client needs.
- A Bachelor's degree in a behavioral sciences field; or a combination of three years' relevant work, education, or experience for QMHA eligibility.
- Has a current Basic Life Support for Healthcare Providers (BLS) training certificate consistent with AHA Guidelines for CPR/ECC, and covers breathing and cardiac emergencies - including CPR, AED, and obstructed airway - for adult, child, and infant patients.
- Demonstrate the ability to build effective working relationships, problem-solve, direct patients to appropriate resources, and manage information while maintaining professional boundaries. Understand the boundaries of your role and effectively communicate them to patients.
- Familiarity with mental health environment and workflow.
- Knowledge of community resources and support systems.
