Summary:
The Supportive Services Coordinator will work closely with the Supportive Services Manager to develop, strengthen, monitor, and evaluate the psychosocial initiatives at NWMS, including Health Equity and Health-Related Social Needs & Depression screening. This position will also work directly with patients and those near them, helping guide them through the cancer care continuum to identify barriers to treatment and assist with access to needed resources to decrease the barriers to care.
Essential Duties and Responsibilities
- Works with the Supportive Services Manager to develop, strengthen, monitor, and evaluate Psychosocial initiatives at NWMS.
- Assists with planning and coordination of programs and their activities.
- Monitor implementation of program policies and practices.
- Works to keep programs on schedule, with stated budgets and functioning smoothly.
- Support program growth and development as necessary.
- Coordinate program communications.
- Schedule and organize program-related meetings and events.
- Create and maintain program records, reports, agendas.
- Facilitate positive relations between the program team, the public, media, and other departments within the organization.
- Serve on clinic committees.
- Organize and assist with conducting community outreach on behalf of the program.
- Identify and address structural and cultural barriers to care.
- Work with providers and staff to increase understanding of the community’s history, culture, and needs as well as the cultural appropriateness of existing approaches and materials.
Direct Client Work
- Help clients and families locate resources in the community to assist with basic needs, financial, insurance, transportation, care needs, Etc.
- Maintain a library of appropriate materials such as a directory of services, service logistics, procedures, and amenities and a bank of application and program eligibility forms.
- Clarify preferences for cultural practices; if appropriate, contact cultural or spiritual leaders to support the client and family with traditional approaches.
- Patient referral as appropriate for patient needs; assists in coordinating patient care.
- Per department documentation standards, provide thorough documentation of all interactions and interventions provided to patients and families.
- Performs various duties as needed to fulfill the function of the position successfully.
- Administer patient surveys and screenings to assess satisfaction, improve care, identify barriers, and elicit patient activation.
- Other duties may be assigned.
Additional duties:
- Promote a proactive and professional relationship with external affiliates/organizations.
- Maintain strictest confidentiality; adhere to all HIPAA guidelines/ regulations.
- Attend meetings and trainings as required.
Supervisory Responsibilities: This position does not have supervisory responsibilities.

Education/Experience
Preferred: Bachelor’s degree in social work or another health-related field. A minimum of 12 months’ experience in a medical or other clinical setting preferred.
Certificates and Licenses: None required.
Computer Skills
To perform this job successfully, and individual should be proficient with standard software programs used in healthcare (such as MS Word, Excel, Outlook) and web browsers, patient registration systems and electronic medical record. Can operate basic office equipment such as telephones, computers, fax and copy machines.
Language Ability
Excellent English communication skills required. Additional language skills appropriate to the patient population served is desirable. Ability to communicate well both verbally and in writing with patients and other members of the clinical team. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence.
Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability
Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

This position is in a standard medical office environment with desktop business equipment and frequent telephone calls. There may be frequent interruptions and high utilization on some days and low utilization on others. The noise level is moderate.
