Responsibilities
The Norton Children’s Prevention and Wellness department is looking for a Prevention and Wellness coordinator that focuses on social determinants of health and current data trends to guide program development, implementation, evaluation and data collection to promote wellness initiatives, injury prevention, safety and chronic disease management to children, families and the community. The coordinator will use National, State and local community assessments to align programming to Norton Healthcare system goals, Norton pediatric primary care initiatives and our internal and external partners. This individual must be willing to work a variable schedule including some weekends and evenings, and be able to travel throughout the community. Excellent communication skills, interest in public health, self-direction and compassion are required.
Qualifications
Required:
- 3 years in outreach or education
- Bachelor Degree
Desired:
- Experience with project management, community outreach, health promotion, and data and grant management. Ability to be bilingual.
- Master Degree
- Certified Diabetes Educator (National) OR Certified Asthma Educator OR Certified Health Education Specialist
