Overview
The Community Coaching Coordinator is responsible for overseeing and facilitating community coaching programs designed to assist individuals with intellectual and developmental disabilities (IDD) in building social skills, increasing community involvement, and fostering independence. The coordinator will work directly with participants, providing guidance and support to ensure the successful achievement of their personal goals in areas like socialization, self-advocacy, life skills, and community integration.
Responsibilities
- Maintain and create calendars for each individual for activities, including community events, workshops, appearances and other communication opportunities.
- Communicate with employees regarding their schedule and scheduled activities.
- Assist in the preparation of monthly budget for community activities.
- Ability to disseminate information clearly and concisely to case manager, parents/guardian, individuals receiving services and others involved in treatment team
- Ability to create and sustain positive working relationships at all levels of the agency; internal and external.
- Manages day to day operations.
- Utilizes effective interpersonal, counseling and negotiation skills to handle sensitive issues with families, individuals, staff and outside agencies.
- Manages employees clocking in and out using the appointed system (Pick up/drop off).
- Communicate with NSS administration as well as other providers any pertinent information that needs to be addressed.
· Build and maintain relationships with local organizations, businesses, and community resources to create opportunities for individuals.
· Conduct necessary assessments for individuals and home visits as required
· Makes referrals and linkages to appropriate agencies for services, coordinates individuals services and treatment with multiple service providers and agencies.
· Demonstrate effective problem-solving and decision-making skills.
· Conduct investigations involving serious injuries and allegations to include making notifications to the Human Rights Office, APS, and completing necessary documents to include CHRIS.
Documentation
· Make corrections on all documentation as deemed necessary by CAO/CEO.
· Oversee submission of billing sheets and progress notes.
· Complete intake packet for new individuals.
· Complete quarterlies for individuals receiving services.
· Create/ Update/ monitor employee charts as required.
Customer Service
- Provide Customer Service to all individuals Families, Guardians, Authorized Representatives, Case Managers, and other outside entities.
- Notifies NSS administration if an individual refuses to attend scheduled coaching session.
- Notifies NSS administration if a Support Coordinator or parent/guardian informs you of issue or concern.
- Follow policies and procedures regarding dress code, cell phone usage, smoking, and visitors.
- As a mandated reporter, employees are required to report all witnessed or suspected abuse, neglect, exploitation, and serious incidents/injuries immediately to Nova Support Services leadership (CEO/CAO). Contact information for Nova Support Services administrator(s) as well as external agencies (APS, Human Rights, etc…)
- Required to participate and follow all procedures related to investigations. This includes investigations conducted by Nova Support Services staff and external agencies, including but not limited to, DBHDS, APS, Law Enforcement, and Disability Law.
Qualifications
- High school diploma or GED required
- Must be at least 21 years of age, have a valid driver's license and acceptable driving record
- Insured personal vehicle
- Willing to use personal vehicle
- Experience in a managing behaviors of individuals with DD diagnosis.
- Satisfactory completion of CPR and First Aid Training Class.
- Satisfactory completion of Medication Management Certification/TOVA.
- Satisfactory completion of Home and Community Based Services Training.
Job Types: Full-time, Part-time
Pay: $16.50 - $19.00 per hour
Expected hours: 5 – 35 per week
