JOB SUMMARY
The Nurse-Family Partnership® (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors are responsible for maintaining the highest standards in clinical nursing practice; adhere to the NFP model, and to the policies, procedures, guidelines and standards of the NFP, and of the Visiting Nurse Association of Southeastern Connecticut (VNASC). Nurse Home Visitors report to the NFP Nurse Supervisor.
Provide home visits to women and their families eligible for the NFP Program
Provide home visits to the NFP service area that includes New London and Middlesex Counties
Adhere to nursing process and the NFP model of home visitation
Maintain a case load of minimum of 25 clients
Provide and document ante/post-partum skilled nursing visits to NFP mothers
Support and implement all policies, procedures, guidelines and standards of NFP and VNASC
As requested/required, participate in community coalition/activities that support NFP
As requested/required, promote public awareness of NFP
Develop and maintain community relationships to support NFP client referrals
Assist with NFP client referral and recruitment activity
JOB QUALIFICATIONS
BSN required
Current Connecticut RN license (in good standing) required
Current adult-children-infant CPR certification required
Two years recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred
Home visiting experience preferred
Excellent written and verbal communication skills
Basic computer skills
Valid driver’s license and insured automobile required
Ability to travel to/from home visits throughout the entire NFP service area
Work a flexible schedule including occasional evenings and weekends (per agency requirements)
Travel, including air, to Denver, CO or other locations for all face-to-face NFP required education sessions
Spanish language speaker helpful.
Typically, Monday-Friday, days, but with flexibility for later afternoon visits to meet client’s needs. Occasional evening or weekend day for special events; otherwise no weekends.
