PURPOSE:
Nurse-Family Partnership (NFP) is a research-based program that has been demonstrated to significantly improve critical outcomes for at-risk first time pregnant parents and their children who received services in local communities as specified in the NFP guidelines. The three major goals of the program are to (a) improve prenatal outcomes, (b) improve child health and development, and (c) improve self-sufficiency of the family.
FUNCTION:
The NFP Nurse Home Visitor (NHV) establishes a trusting, therapeutic one on one relationship with low income, first time pregnant persons, who voluntarily consent to regularly scheduled visits starting in pregnancy and continuing until the child’s second birthday. The NHV provides comprehensive nursing services utilizing evidence based assessments and clinical nursing judgement to assess the physical, emotional, social, and environmental needs of the family. The NHV implements standard interventions including guidance, health education, referral and case management in areas including prenatal, antenatal, and post-partum care, nutrition, parenting, child health and development, family planning and life course development. Motivational Interviewing is the fundamental base of the therapeutic relationship between the nurse and client that is pivotal in transforming the lives of vulnerable babies, parents, and families. This therapeutic tool supports the NHV in empowering the client to identify, clarify, and obtain the skills needed to accomplish their goals.
REPORTS TO:
Care Team Manager, Monroe County Nurse-Family Partnership Supervisor
RELATIONSHIPS:
Adult client, infant/child, family and significant others, other NHVs and team members, NFP Supervisors, NFP Program Coordinators, other community based organizations, physicians and health care professionals. NFP National Service Office team, Community Advisory Board, stakeholders.
COACHES:
Students and Interns as necessary
RESPONSIBILITIES:
Provides home visits to a caseload of 25-30 families adhering to nursing process and the NFP model of home visitation.
Establishes a therapeutic relationship with clients and their families that promotes self-efficacy.
Assesses physical, emotional, social, and environmental needs of clients and their families as they relate to the well-being of the family.
Works collaboratively with client and their families to establish and attain measurable goals in a client centered and inclusive manner.
Establishes individualized plan of care utilizing clinical nursing judgement and standardized NFP assessments.
Refer families to community resources as needed.
Consults and collaborates with other professionals and community agencies actively engaged with clients and families receiving NFP services.
Documents all nursing care and interactions in accordance with Monroe County Nurse-Family Partnership Protocol.
Participates in ongoing Continuous Quality Improvement efforts.
Assumes responsibility for ongoing learning and professional development within NFP practice.
Performs related duties as assigned or required.
QUALITIFICATIONS:
Skill and Knowledge
Baccalaureate degree in nursing.
Current NYS nursing license in good standing required
Two years recent experience in maternal/child health, public health/home visiting or mental health/behavioral health nursing preferred.
Ability to establish interpersonal relationships with individuals and families from diverse socioeconomic and cultural backgrounds.
Excellent written and verbal communication skills.
Basic computer skills, electronic health record experience preferred.
High degree of autonomy. Ability to impose structure throughout the workday; both in between and during visits.
Ability to speak Spanish is highly desired.
Working Conditions
Work assignments require daily travel throughout Monroe County and ability to report daily to office in Rochester, NY.
Able to complete duties under stress, deadlines and while attending to multiple tasks simultaneously.
Must have access to an operable automobile and have a valid driver’s license and insurance.
Must be able to do light to moderate lifting, climb stairs. Workday can include extended periods of sitting and/or standing.
