Key Responsibilities:
- Conduct assessments to identify clients' psychological, social, and medical needs.
- Collaborate with clients and families to develop and adjust personalized care plans.
- Connect clients with community resources to enhance their well-being.
- Provide support during discharge planning and case conferences.
- Maintain accurate and timely documentation in line with agency policies.
- Supervise social work assistants as needed and ensure the delivery of quality care.
- Performs other duties as assigned
Qualifications:
- Graduate of an accredited college with a Master of Social Work degree or equivalent
- Current state licensure.
- Minimum one year of supervised or equivalent work experience (home care experience preferred).
- Strong communication, organizational, and problem-solving skills.
