Key Responsibilities:
- Provide clinical oversight and leadership for the agency’s home health services.
- Manage and support nursing development across the team, ensuring high standards of care and compliance.
- Serve as a mentor and educator for new and existing staff, fostering a culture of learning and clinical excellence.
- Oversee orientation, training, and competency evaluation of field clinicians.
- Collaborate with the administrative team to develop and implement clinical policies and procedures.
- Monitor and analyze clinical performance metrics, ensuring positive patient outcomes.
- Participate in interdisciplinary case reviews and care planning as needed.
- Ensure adherence to all federal, state, and local regulations, as well as accrediting body standards.
- Act as a clinical resource and liaison for staff, patients, families, and providers.
- Support ongoing agency growth and program development through clinical innovation and quality initiatives.
- Active RN license in the state of Connecticut (required).
- Minimum of 3 years of home health experience, with at least 1 year in a leadership or supervisory role.
- Strong knowledge of Medicare regulations, OASIS, and clinical documentation standards.
- Proven ability to lead, mentor, and develop nursing staff in a home health environment.
- Excellent organizational, communication, and interpersonal skills.
