We are a home care services provider seeking a Sales Professional to develop and establish community relationships with hospitals, ALF, Hospice agencies, Home Healthcare agencies, Community partners, nursing homes, Clinics, and physician offices in an effort to grow our patient census and ensure that our community’s care needs are met.
Essential Job Functions/Responsibilities
- Consult and collaborate with physicians, nurses, social workers, discharge planners, and other disciplines to establish a coordinated plan of care.
- Meets with the patients, families, and caregivers to discuss the home situation and any medical/psychosocial factors that are relevant to the plan of care.
- Completes referral information that includes intake data, essential background information, hospital course, and plan of care.
- Establishes and maintains working relationships with hospitals, ALF, Hospice agencies, Home Healthcare agencies, Community partners, Aging Life Care Managers, Social Workers across organizations, nursing homes, Clinics, and physician offices to increase community engagement.
- Collects and maintains statistical data on all referrals and submits them regularly as required.
- Works closely with Clinical Managers and clinical team members to provide the best possible patient continuity and outcomes.
- Meets performance standards set forth by the Administrator.
- Performs other duties as assigned.
Position Qualifications
- Clinical or Healthcare Background Strongly Preferred
- Experience in Homecare/home health, healthcare, social worker or case manager preferred
- Experience meeting goals, deadlines, and performance benchmarks.
- Demonstrates an ability to work with other health care clinicians and can have difficult goals of care conversations with patients and families.
- Self-directed and able to work with minimal supervision.
- Strong ability to cultivate new relationships.
· Excellent written and oral communication skills
· Ability to be a vital contributing member of a team
· Work independently and be a self-starter
