About the Role: We are seeking a dedicated Wraparound Care Coordinator from Martinsburg, WV to join our team. The Coordinator will work closely with children and families enrolled in the Children with Serious Emotional Disorder Waiver (CSEDW) program. You will be responsible for developing and managing personalized care plans (POCs), coordinating with service providers, and ensuring the delivery of high-quality services that meet the unique needs of each child.
Key Responsibilities:
- Engage with members and their families to develop and implement individualized care plans.
- Assemble and lead Child and Family Teams (CFTs) to identify strengths, needs, and resources.
- Conduct the CANS assessment, a trauma-informed behavioral health evaluation tool.
- Monitor the implementation of care plans, ensuring services are delivered effectively.
- Facilitate connections with service providers, natural supports, and community resources.
- Conduct regular in-home visits to assess progress, verify service delivery, and address any issues.
- Coordinate discharge and after-care planning, ensuring a smooth transition for members.
- Maintain detailed documentation of care plans, assessments, and all interactions with members.
Requirements:
- Bachelor’s degree in human services or a related field.
- Minimum of 2 years post-college work experience in human services.
- Certification to perform the CANS assessment.
- Completion of training in Wraparound Facilitation and National Wraparound Initiative (NWI) principles.
- Must be a fully licensed social worker, professional counselor, or registered nurse in West Virginia, or have equivalent qualifications.
- Strong knowledge of community resources and inter-agency coordination.
- Ability to work independently and as part of a team, with excellent communication and organizational skills.
- Must be a certified and approved provider under West Virginia Medicaid and CSEDW.
Preferred Qualifications:
- Experience working with children and families in mental health or social services.
- Previous experience with the CSEDW program or similar initiatives.
