What are the responsibilities and job description for the Well-Being Specialist position at NWF Health Network?
POSITION OBJECTIVE:Promote the concept of well-being as an integral component of Child Safety and Permanency. This position provides oversight, coordination, and support to Case Management Organizations within their circuit to assure quality of educational, medical, dental, behavioral health and human trafficking services. Monitor data integrity and develop resources as necessary.
ESSENTIAL FUNCTIONS:
· Facilitate educational meetings in compliance with the Every Student Succeeds Act to ensure educational stability for children that are sheltered and/or change placement while in care.
· Serve as point of contact for school district to assist case management with coordination of transportation, obtaining educational records, and identifying and coordinating additional support for children in care as needed.
· Track the academic performance of all foster youth (in licensed care) ages 15 and above, ensuring that they are linked to the educational programs needed to get or remain on track for graduation. Review diagnostic educational assessments to determine grade readiness and any educational gaps. Review and monitor child-specific educational plans.
· Track the academic performance of all youth in extended foster care (ages 19 to 21) who are working on completing credits for a high school diploma or GED.
· Assist with identification and enrollment of appropriate foster youth in the GED program.
· Serve as Human Trafficking subject matter expert and lead for assigned circuit
· Attend all Human Trafficking staffings and Task Force Meetings; assist case management in identifying and serving minor victims of Human Trafficking.
· Facilitate regular staffings for youth in CSEC placements to review treatment progress and service availability and ensure effective discharge planning and promote motion toward least restrictive settings.
· Coordinate with local providers to ensure youth victims of sex and labor trafficking are provided comprehensive, coordinated and trauma-informed services to fully address their needs.
· Coordinate with DCF and local children services agencies to create community-based human trafficking protocols to effectively respond to suspected cases of both sex and labor trafficking;
· Coordinate with DJJ local work groups to develop and implement sustainable cross-over protocols for identifying and serving minor victims of human trafficking.
· Track human trafficking victim data locally, and partner with DCF statewide human trafficking data-collection efforts.
· Maintain a program of professional development, including any requirements necessary and training hours to maintain licensure.
· Participate (as needed) in high-risk community child(ren) staffings facilitated by the Managing Entity.
· Collaborate with the nurse coordinator to facilitate staffings with physicians, dentists, and educational leaders to problem solve and address any issues related to service provision or information sharing.
· Coordinate and provide project management for Well-being initiatives.
· Comply with all personnel policies & procedures.
· Participate in community activities such as ad hoc committees and work groups.
· Participate in the Quality Assurance/Quality Improvement process.

QUALIFICATIONS:
· Bachelor’s degree from an accredited university and 5 years of experience in a human services field OR
Master’s degree from an accredited university and 3 years of experience in a human services field
- Child Welfare Certification issued by the Florida Certification Board (or be eligible for and willing to achieve certification within six months of hire date). (See attached exhibit for requirements for certification.)
· Child Protection Professional Certification – Prior certification or significant employment history with our targeted population may substitute.
· Prior supervision experience
· Master’s degree in a human services field
· Previous experience with the Independent Living Program
· Prior experience with FSFN and related reporting
