What are the responsibilities and job description for the Transitions Coordinator position at Nye Health Services?
Position Summary
The Transitions Coordinator is responsible for developing and implementing census growth strategies within market in order to maintain and grow referral relationships, generate new referrals, and meet census goals. They will engage all new referrals, promoting campus clinical capabilities and serve as a liaison between campus and referral source to ensure all transitions to campus are completed safely and successfully.
In this role you will:
- Assist in identifying, overseeing, and facilitating marketing opportunities for referring hospitals, physicians, and all other referrals sources.
- Conduct pre-placement screens of potential admits to assist in determining appropriate placement of patients.
- Develop and maintain admissions and census growth strategies for the campus, conducting monthly census development meeting.
Qualified candidates will have:
- Bachelor’s Degree and/or valid RN/LPN license from appropriate state preferred.
- Experience and/or training in Long-Term and Acute Care environments.
- Experience in working collaboratively with physicians, discharge planners/case managers and customers.
