Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program. Position Summary :

We have an exciting opportunity to join our team as a Counselor.

In this role, the successful candidate The Counselor is responsible for providing case management support for community members and participants at the Family Support Center, as well as facilitating psycho educational workshops. .This includes advocacy, assessment, benefit screening and resource screening, connection to resources, and benefit application assistance. Excellent engagement skills and ability to work with individuals and families in crisis.

Job Responsibilities :

• Conducts outreach and educational workshops
• Other duties as assigned by Family Support leadership.
• Interviews clients to assess needs for social services.
• Attend staff meetings and other appropriate meetings provided by the agency.
• Identifies and makes referrals to a variety of public and community agencies providing food shelter clothing medical educational and other services schedules client appointments acts as client advocate in obtaining services.
• Manages caseload, prepares narrative and statistical reports, and documents correspondence regarding client status.
• Provide advocacy around SNAP and other public benefits and resources.
• Provides short term supportive counseling to clients.
• Documents and tracks work, including referrals, SNAP applications, outreach and case management.
• Provides case management and support for SNAP eligibility screenings, applications and re-certifications.

Minimum Qualifications :

To qualify you must have a High School Diploma or equivalent required.

Bachelor's Degree preferred.

1 years relevant experience working, ability to plan and manage,

Knowledge of public benefits

History of immigrant community engagement

Good interpersonal skills

Attention to detail

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $41,137.15 - $50,562.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses / incentive, differential pay or other forms of compensation or benefits.
