What are the responsibilities and job description for the Social Services / Admissions Coordinator position at OAK WOODS OF MER ROUGE?
- Collecting all appropriate medical and financial information pertinent to an admission decision.
- Collecting all appropriate medical and financial information pertinent to the Long Term Care Medicaid application.
- Assisting and communicating with all insurance companies, and completing any insurance priauthorizations.
- Ensuring that proper documentation is sent to all necessary departments prior to patient admission.
- Coordinating all admitting department operations in a nursing home environment.
- Ensuring compliance with applicable standards.
- Overseeing the in-patient/out-patient functions, bed assignments, and completion of preliminary paperwork for entering patients.
- Assisting patients when they discharge from the facility and set up outpatient needs, such as home health, in-home equipment.
- Assisting residents upon the time and decision making on enrolling in hospice.
- Conducting psychosocial assessments for new residents.
- Developing individualized care plans in collaboration with healthcare teams.
- Providing individual and group counseling to residents.
- Keeping an adequate facility greivance log and aiding in resolving greivances
- Assisting residents in adjusting to nursing home life.
- Advocating for residents' rights and needs.
- Navigating the nursing home system and understanding residents' rights.
- Developing and implementing programs to improve residents' quality of life.
- Supporting residents' well-being and addressing immediate and long-term needs.
