Oakdale Seniors Alliance is seeking a dedicated Admissions Coordinator to join our team! In this unique role, you'll bridge the gap between our exceptional clinical services and assigned communities, ensuring healthcare providers and potential residents understand the full range of care options we offer.
What You'll Do :
- Assist Skilled Nursing Facilities build census by working with referral partners to identify appropriate candidates for admissions.
- Conduct patient clinical referral assessments, identifying appropriate skill-able needs and offer skilled nursing services and opportunities for admission.
- Customer service support, rapport building, Q&A, timely engagement, skilled service and insurance knowledge.
- Engage referral partners, patients and families via phone and computer applications regarding their healthcare needs and concerns.
- Serve as first point of contact between referral partners, patients, families, and healthcare providers.
- Remote position working independently and within a central team.
