About The Opportunity/Work

The Wraparound Facilitator for the Specialized Services for Youth department provides facilitation of core elements of the Wraparound model through the Child and Family team process, working with families enrolled in the program for youth with serious emotional disturbances. The Wraparound Facilitator works in collaboration with the Child and Family Team to identify formal and informal supports, complete strengths assessments, create a cost-effective Person-Centered Plan, crisis support plans, and is responsible for monitoring of the developed plan. The Wraparound Facilitator meets regularly with the Child and Family Team and assists families to achieve their goals. This position reports to the Wraparound Supervisor in the Specialized Services for Youth department.

  • Facilitates and organizes the Child and Family Team meetings in adherence and fidelity with the Wraparound model.
  • Facilitates completion of the Wraparound Plan, that includes completion of the strengths/culture discoveries, needs assessments, crisis/safety support plans, Wraparound plans, outcomes, the development of the family mission statement and overarching goal, individual and family outcomes, and all services supports and interventions, with input of Child and Family Team.
  • Reviews Wraparound Plan regularly with Child and Family Team and updates in adherence to fidelity with the wraparound model.
  • Provides information and education on topics such as communication and child development.
  • Advocates for youth and family by accompanying them to appointments related to goals/outcomes.
  • Assists youth and family with locating and utilizing community resources and activities.
  • Provides services and support to parents/caregivers to improve family quality of life.
  • Develops and maintains relationships with funded partners, community providers, various community facilities and stakeholders. Maintain frequent contact and collaboration with points of contact for child (school, primary care doctor, therapist, Court, etc). and invite them to Wraparound Team meetings as appropriate.
  • Participates in case coordination and case consultations with the treatment team and other partners.
  • Expect to work at minimum two evenings per week to accommodate school and work schedules.
  • Provides strong fiduciary oversight of the use of any flexible funds in support of the family.
  • Presents or supports families in presenting their plans to the Wraparound Community Team.
  • Participates in professional development activities in accordance with state licensure requirements and any requirements of the funder, including regular supervision.
  • Operates as the primary case holder for SED-Waiver individuals, that includes conducting a comprehensive assessment of individuals’ non-treatment needs, developing case management aspects of Individual Plan of Service/Family Plan of Service (IPOS/FPOS), coordinating services on the IPOS/FPOS by linking individual/family with community based-services, coordinating with other service providers and assisting in out-of-home placements (ie. in-patient hospitals or Children’s Village) as appropriate.
  • Maintains clinical records in accordance with Agency, licensing, accreditation, Community Mental Health, Michigan Department of Community Health standards and requirements.
  • Responsible for incorporating principles of diversity, equity and inclusion in work processes and in job functions. Responsible for promoting a feeling of welcoming, belonging, and acceptance to all clients and staff.
  • Other duties as assigned.

Does this Describe YOU?

  • Bachelor’s degree in social work or related field with appropriate state licensure.
  • Minimum of three years of experience working with children and families required.
  • Have or develop significant understanding of clinical and non-clinical systems in Oakland County, including courts, schools, Department of Community Health, Department of Human Services, respite systems, Community Mental Health, community living supports, etc.
  • Other special abilities, skills and credentials such as experience working in the Community Mental Health System, substance abuse qualifications, experience with youth with serious emotional disturbances, family systems work, trauma-informed care, delivery of home-based models and use of evidenced based models.
  • Selected candidates for this position must possess a sensitivity to the diversity of the agency’s service population including (but not limited to) differences of culture, race, religion, gender, sexual orientation and socioeconomic.
  • Special abilities and skills necessary to perform the required tasks and that best meet the needs of the agency also will be considered. There may be some standards above that may be waived when compensating specifications or circumstances exist.
  • Although employees have use of the Agency vehicle fleet to conduct business and are covered under our insurance, employees must provide documentation of personal vehicle liability insurance with minimum coverage of $100,000 per person/ $300,000 per incident. Employees also must provide documentation of a valid driver’s license on an annual basis as well as proof of registration. In some instances, depending on the nature and frequency of driving, a chauffeur’s license must be obtained. (may obtain within sixty days of hire).

Salary : $43,500 - $52,400
