Responsibilities:
- Conduct assessment and intake to understand the needs of individuals served.
- Jointly create a Recovery Plan with the individual to support them in achieving their goals.
- Link individuals to community supports including, but not limited to, legal assistance, housing opportunities, rental assistance, education opportunities, employment opportunities, vocational programs, mainstream benefits, primary care and behavioral health services, daycare, etc.
- Assist individuals in strengthening their skillsets through coaching, role-modeling, instruction and in-person support.
- Provide services “in-vivo.”
- Ensure that services provided are recovery-based, trauma informed and culturally responsive to individual served.
- Collaborate with other team members and communicate regularly to promote team-based care and shared-decision making.
- Provide training to clients on the recognition of relapse triggers and strategies to deal with avoiding a relapse.
- Demonstrate in-depth knowledge of the community.
- Advocate on behalf of the individual by providing a holistic experience
- Encourage and actively solicit feedback from individuals served to promote person-centered services.
- Provide in-person and telephone support to individuals.
- Document all in-person, collateral and phone contacts in an electronic health record within 24 hours of the service being rendered.
- Participate in Team Meetings and individual supervisions as schedule.

Qualifications:
- Bachelor's degree with CADC, LCADC; OR
- Master's degree in Health, Psychology, Counseling Education or other behavioral health profession
- Valid Driver's License.
