POSITION OVERVIEW
The Assisted Living Nursing Director provides and coordinates the overall health monitoring and nursing related services to residents in compliance with Chapter DHS 83 of the Wisconsin Administrative Code while promoting wellness and independence. The Assisted Living Nursing Director (1) provides leadership and supervision to the nursing and resident assistant staff in the assisted living programs; (2) provides overall leadership for assessment, planning, implementation and evaluation of resident care; (3) Collaborates with interdisciplinary staff on issues related to resident care and the quality of life for all residents; (4) monitors compliance with DHS 83 regulations and follows up on changes in condition; documentations; incidents/accidents, and risks of infection, falls and confusion; and (5) investigates and completes the Assisted Living Facility Self Reports (F-02208) to DQA BAL (6) assists with the investigation and completion of any Misconduct Incident Reports to DQA BAL (7) provides general support 24/7 for residents, families and staff via in-person, on-call, or email. Accepting 24-hour per day accountability for clinical cares delivered, the Assisted Living Nursing Director collaborates with the care delivery team both internally and externally, acting as the clinical liaison to acute care providers, physicians, families, and residents/clients.

ESSENTIAL RESPONSIBILITIES (including but not limited to)
· Ensures the delivery of quality services, and actively listens to residents/families’ grievances/concerns to ensure prompt resolution and satisfaction.
· Provides oversight for systematic programs.
· Reviews initial clinical assessments of all prospective residents to include the determination of the appropriate household, level of care and services provided, and the timeline for an effective move-in.
· Oversees development of initial Individual Service Plans (ISP) for each resident upon admission and in conjunction with the AL interdisciplinary team, finalizes a comprehensive ISP within 30 days. Ensures completion of annual and ongoing ISP reviews.
· Attends or delegates Care Plan conferences within 30 days of admission and as needed.
· Provides leadership to the nursing/supervisory staff to perform assessments, planning, implementation, and evaluation of all nursing services to all residents of the facilities.
· Provides oversight and resources for education to Nurses (RNs/LPNs), Supervisors, and Resident Assistants. This includes group presentations and one-on-one mentoring/training. Recommends ongoing staff development programs to assure development and learning of current practices as it relates to geriatric care (general training/re-training of staff).
· Assists in staffing functions, including interviewing, hiring, scheduling, coaching and corrective action as needed or as delegated.
· Recommends, collects data, and evaluates QA data including falls, wounds, psychotropic med review, infection control, med errors, and any pertinent Quality Assurance measurable areas. Works with pharmacy to ensure medication protocol is followed.
· Ensures maintenance of records and charts to reflect residents’ conditions, delegates charting responsibilities to Nurses (RNs/LPNs), Supervisors and Resident Assistants as needed, and where appropriate.
· Collaborates, develops, updates, and maintains consistent policies with other CBRF’s as it relates to nursing and health services to ensure current and meets regulations.
· Maintains regular communication with Executive Director, Social Worker(s), Physicians, Pharmacist, residents and families/ responsible parties, and other vested parties.
· Consults with other Clinical leaders as needed.
· Supervises the applicable Shift Supervisors to include AM, PM, NOC, and any other pertinent Supervisory staff for the Assisted Living facilities.
· Ensures that all Oakwood policies are administered effectively and accurately. Ensures that health and nursing policies are current and meet regulations.
· Holds accountability for remaining updated with any changes in the State of Wisconsin, Chapter 83, and that the CBRF regulations are being followed.
· Attends meetings and participate on committees as assigned or as it relates to this position.
· Facilitates regular review of program and care services.
· Available for flexible hours and 24/7 on-call oversight as needed.
· Provides “back up” to support other areas on the respective campuses.
· Participates in Leading Age Echelon and the required WCCEAL components.

EXPERIENCE, EDUCATION, AND/OR TRAINING
· Graduate of an accredited nursing school with a BSN and registration or eligible for registration as an RN in the State of Wisconsin.
· Experience caring for geriatric individuals is required.
· Prefer at least three to five (3-5) years of progressive experience working in long-term care or health related field, part of which is in a supervisory, leadership and/or training
capacity.
· Completed all CBRF courses and/or approved administrative course.
· Computer skills required.
