Role Description
We are seeking a dedicated and motivated Housing Coordinator to join our team. The ideal candidate will manage a caseload of 10 clients, providing comprehensive support to secure and maintain stable housing. This role requires strong organizational skills, excellent communication, proactiveness to better client livelihood, and a deep understanding of housing resources and benefits. This will be a hybrid role, meeting with the larger team on Mondays/Wednesdays in-office.
Minimum Qualifications
- 1-3 years experience in human and/or social services (housing experience preferred)
- 1 year experience working with vulnerable/disabled adults
- Reliable vehicle, valid driver's license, and ability to transport clients (REQUIRED)
- Good knowledge of Twin Cities metro area
- Good knowledge of Benefits Housing resources in Twin Cities metro area
- Ability to work independently when needed, and collaboratively with the team as well
- Must pass criminal background check
- Commitment to maintaining confidentiality and adhering to HIPAA and Data Privacy Practices.
Preferred Qualifications
- Housing Stabilization Services experience (HIGHLY preferred)
- Bachelor's degree in Social Work, Human Services, or related field is preferred
- Strong Communication skills for interaction with clients and staff
- Proficiency in Case Management and Supportive Housing services
- Excellent organizational and problem-solving skills
- Ability to work collaboratively and independently
- Experience in working with diverse populations is a plus
Responsibilities:
- Manage a caseload of 10 clients independently.
- Record visit notes daily with professionalism, clearly defining purpose and result of respective visits.
- Identify and address barriers to housing access for clients.
- Perform need assessments and develop support plans aimed at stabilizing and sustaining client housing needs.
- Search for housing and establish relationships with landlords, property managers, and owners.
- Conduct regular in-home and community visits to evaluate client progress and provide support.
- Collaborate with the management team, sharing ideas and feedback to enhance team performance.
- Expand knowledge of local and state resources to secure additional income assistance for clients.
- Assist clients in applying for benefits to help cover housing costs.
- Maintain professional and positive communication with case managers, providers, family members, and others from diverse backgrounds.
- Navigate the processes of Minnesota Healthcare programs.
- Arrive in office at least 2 times a week. Monthly meetings, organized with the coordination of HSS managers and team availabilities, which are mandatory.
- Keep accurate files and program records.
- Adhere to HIPAA and Data Privacy Practices.
