Client Services Advocate (Case Manager) -Direct Services

Openings: (2) Ocean County, & (2) Atlantic / Cape May County

35 hours / week $18-$27/hour based on education and experience

· Client Assessment: identifies and addresses immediate and long-term needs.

· Case Management: coordinates services to ensure wellbeing, stability, health care, safety and avenues for self-sufficiency of clients.

· Application Support: assists clients with the completion of documents for income eligible programs, assessing employable skills and abilities, housing stabilization, child care assistance.

· Client Documentation: maintains accurate client records while developing and maintaining working relationship with other agencies including Mental Health Facilities, Public Defenders, Social Security Office, DCP&P, Board of Social Services, and more.

· Follow-up Services: Documentation, interventions, data input and outcomes are required regularly. Organizes and participates in community outreach to educate the broader community. Available to work outside of normal business hours when necessary.

Education and skill requirements:

· BA / BS in Social Work, Psychology, or related field with professional

experience in case management. Will consider candidates with AA and solid organizational skills.

· Proficient in all Microsoft applications and adapt quickly to agency software and databases.

· Effective and relatable communication with clients and staff of diverse backgrounds.

· Prefer candidate to speak, read and write Spanish in addition to English to meet caseload need.

· Valid Driver's License and transportation. Must be able to lift at least 45 pounds.

Job Type: Full-time

Pay: $18.00 - $27.00 per hour

Expected hours: 37 per week
