MAJOR FUNCTIONS :
The Case Manager is responsible for substance abuse counseling and intervention services. Must be able to stabilize and promote self-sufficiency and optimum health for people living with HIV / AIDS, Mental illness and / or substance use disorders. Manage a case load of 20 to 25. Provide advocacy as well as coordinate services through home visits.
SALARY : $30,000 - $45,000 Annually
SPECIFIC DUTIES & RESPONSIBILITIES :
REQUIREMENTS : EDUCATION AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES : High School Diploma, one to two years of experience in counseling or two (2) years of Odyssey House experience and a High School Diploma, or Bachelor’s Degree in Human Service.
