The HCBS/DDD Referral Coordinator is responsible for managing and coordinating referrals for Home and Community-Based Services (HCBS) within the Division of Developmental Disabilities (DDD). This role ensures timely response to service requests, effective communication with referral sources, and adherence to compliance and regulatory requirements. The Referral Coordinator plays a vital role in streamlining the intake process to ensure individuals receive appropriate services efficiently.

Essential job functions
Referral Management:
- Receive and process HCBS referrals from DDD, families, case managers, and other sources.
- Review and verify referral documentation for completeness and accuracy.
- Assign referrals to the appropriate provider(s) based on availability, qualifications, and client needs.
- Track and maintain a database of referrals, service status, and outcomes.
Communication & Coordination:
- Serve as the primary point of contact for referral sources, ensuring clear and timely communication.
- Collaborate with families, case managers, and providers to facilitate service initiation.
- Provide updates to stakeholders regarding referral status, delays, or additional documentation needed.
- Coordinate with internal teams to ensure smooth onboarding of new clients.
Compliance & Documentation:
- Ensure all referrals comply with HCBS and DDD regulations, policies, and procedures.
- Maintain accurate records in electronic systems and ensure timely documentation of all interactions.
- Assist in preparing reports on referral trends, service utilization, and provider capacity.
- Monitor compliance with provider enrollment requirements and service authorizations.
Process Improvement & Support:
- Identify opportunities to improve the referral process and reduce service gaps
- Assist in developing workflows and strategies to enhance efficiency in referral management.
- Support recruitment efforts by identifying areas where additional service providers are needed.
- Participate in team meetings and contribute to problem-solving discussions.
- Knowledge, Skills and Abilities

Education & Experience:
- High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred.
- Experience in HCBS, DDD services, case management, or healthcare referral coordination preferred.
- Familiarity with DDD eligibility, service authorization, and HCBS program guidelines
Skills & Competencies:
- Strong organizational and time management skills.
- Excellent verbal and written communication skills.
- Proficiency in Microsoft Office (Word, Excel, Outlook) and database management.
- Ability to multi-task and work in a fast-paced environment.

Working Conditions
- Office-based with potential for remote work or occasional site visits.
- Requires sitting for extended periods and using a computer.
- Occasional travel for meetings, training, or site visits may be required.

- Travel to remote territories and within the community ongoing and on a regular basis
