OACHC is the primary source for training and technical assistance (T/TA) for all 60 of Ohio’s Community Health Centers.
PRIMARY OBJECTIVES:
Working directly with the Director of Value Based Programs, the Manager of Value Based Programs (MVBP) is responsible for managing, supporting, and implementing value-based care programs with member FQHCs. Additionally, the MVBP serves as a technical resource for FQHC data reporting and visualizations; value-based care program management including: contract crosswalks, expansion of value-based care program, monthly networking calls, practice transformation and alignment with value-based models of care and focus on total cost care goals. As a leader, the MVBP supervises the Practice Transformation Specialists.

Additionally, proven ability and experience with building and maintaining internal and external highly effective working relationships is key to success in the Manager of Value Based Programs role. The individual must be self-motivated and able to manage multiple tasks, work with diverse people, carry a heavy workload, and travel. Demonstrated proficiency in health care services and quality improvement and awareness of, and value for, cultural competence. Success will be evident in the ability to work autonomously with a highly diverse population. The ideal MVBP will display sound judgment, relate well to the public and staff, have a professional demeanor, prioritize customer satisfaction, and be well organized.
EDUCATIONAL AND EXPERIENCE REQUIREMENTS: Bachelor’s degree (master’s preferred) in appropriate field (public health, health administration, or other health related field). At least two years of experience in the fields of health, public health, or social service. Must have experience in project management and knowledge of value-based care and quality Improvement. Strong preference given to candidates with the following experience: supervision, previous FQHC experience, value-based care experience, quality improvement work, PCMH and/or PCMH recognition programs in a medical setting. Familiar with the National Committee for Quality Assurance (NCQA) standards, as well as other recognition entities such as the Accreditation Association for Ambulatory Health Care and/or the Joint Commission is preferred. Quality Improvement Certification is highly desirable.

MAJOR RESPONSIBILITIES:
· Value Based Care Implementation
o Provide support and assistance with implementing performance improvement tools and resources.
o Support value-based care initiatives by directly working with health centers to increase metric compliance.
o Act as a resource on value-based care for health center and OACHC staff.
o Work collaboratively with OACHC staff and associated stakeholders to understand value-based care contracts. Responsible for translating current and future contract components to health centers and practice transformation team in overview terms. Including creation and maintenance of Contract Crosswalks.
o Maintain and advance Value Based Care program implementation with individual health centers and as a collective through monthly networking calls. Understand the Ohio Comprehensive Primary Care (CPC) program and Ohio Comprehensive Maternal Care (CMC) Programs and be able to assist health centers in understanding the overlap with other value-based-care contracts.
o Responsible for maintaining and creating value-based care action plans with health centers to achieve quality and program incentives.
o Responsible for maintaining and creating total cost of care initiatives with individual health centers.
o Plan and coordinate regular and ad hoc meetings with individual health centers related to value based care.
o Attain working knowledge of Ohio value-based care contracts and initiatives.

o Provide written progress reports and practice narratives as required.
o Develop effective relationships and ensure communication with partners and other key stakeholders (including but not limited to state agencies, project related coalitions, etc).
o Plan and facilitate internal and external meetings, trainings, webinars, and conference calls.
o Share best practices and successes among CHCs through regular communications (e.g. updates, memos to site contacts, meetings with participating sites, conference calls).
o Coordinate with other quality improvement staff on training and technical assistance planning, implementation, delivery, and reporting.
o Perform related responsibilities as assigned by the Director of Value Based Programs
