Responsibilities:
As directed by a Case Manager, you will provide end-of-life care and various services for hospice patients across multiple counties, observing, reporting, and documenting patient status, while offering empathetic support to patients and their families.
Requirements:
- CNA certification
- 1-2 years of experience as a CNA or hospice aide
- Knowledge of hospice aide practices and procedures preferred
- High school diploma or equivalent
- Reliable vehicle, valid driver's license, insurance, and acceptable driving record
