We are seeking a Healthcare Navigator to join our Tulsa team!
About Us: Oklahoma Veterans United is built on an 80 year legacy of bringing our community together to research, plan, coordinate action, and assess progress to address some of Oklahoma’s most critical social service, health, education, and civic challenges. With a fast and ever-changing landscape of nonprofits and funders, we strategically shifted our focus to solely concentrate on the Veteran community in the state of Oklahoma.
In 2024, we officially transitioned from Community Service Council to Oklahoma Veterans United (OKVU). Our organizational focal points are housing services, suicide prevention resources, and employment opportunities for Veterans through key initiatives such as Supportive Services for Veteran Families (SSVF), Staff Sergeant Parker Fox Suicide Prevention Grant Program (SSG FOX SPGP), Veteran Employer Network, Right Face, and more.
As the Healthcare Navigator, you are primarily responsible for working with the local VA Medical Center, and other healthcare systems within the Oklahoma Veterans United SSVF catchment area to ensure appropriate coordination for all Veterans enrolled.
What You Will Do
- Provide assessments to SSVF Case Managers to identify barriers to health care.
- Assist veterans with obtaining access to physical and/or behavioral health services through the VA or a community provider.
- Interface with the VA Medical Center to ensure appropriate care coordination.
- Interface with other healthcare systems to ensure appropriate care coordination.
- Support health care planning by identifying barriers to care.
- Provide education on wellness related topics to the veteran when appropriate.
- Provide advocacy for the veteran with providers across all healthcare systems.
What You Will Bring
- A High School Diploma or equivalent
- Ability to quickly build rapport with veterans from various backgrounds
- Knowledge of community referral resources
- Good computer skills, including Microsoft Word and Excel
- Possess good verbal and written communication skills
