Key Responsibilities :
Assist in implementing individualized treatment plans under the supervision of a licensed Occupational Therapist (OT).
Work with patients to improve their ability to perform daily activities and achieve greater independence.
Provide direct patient care in the home setting, including therapeutic exercises, functional activities, and adaptive equipment training.
Monitor patient progress and communicate observations and updates to the supervising OT.
Educate patients and their families on home exercise programs and safety strategies.
Ensure accurate and timely documentation of patient progress, treatments, and outcomes.
Collaborate with the multidisciplinary team, including physicians, nurses, and case managers, to provide comprehensive patient care.
Adhere to all regulatory and compliance standards in home health therapy services.
Qualifications :
Current and valid COTA certification.
Minimum of 1-2 years of experience as a Certified Occupational Therapy Assistant, preferably in home health.
Knowledge of home health care procedures, patient assessment, and therapy techniques.
Strong interpersonal and communication skills.
Ability to work independently with a strong sense of responsibility and initiative.
Compassionate, patient-centered approach to care.
Familiarity with electronic health records (EHR) is a plus.
