SCHEDULE: Full Time, Monday- Friday; will rotate between our 2 clinics in West Sacramento
PAY RANGE: $26.52 to $32.42 hourly

Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. JOB SUMMARY: The Quality Improvement Coordinator works as part of a multi-disciplinary team to ensure CommuniCare’s patient population receives quality care
EDUCATION, EXPERIENCE, TRAINING
- Medical Assistant certificate or college level coursework with clinic/medical knowledge through a combination of training and experience.
- Administrative experience preferred.
- Experience working in a clinic setting and a demonstrated commitment to comprehensive care for low income clients.
- Bilingual English/Spanish preferred.

DUTIES AND RESPONSIBILITIES
1. | Works with the current Hansen PCC as the Quality Improvement Program Leads for Hansen Clinic. Monitors QI measures at Hansen Clinic continuously and identifies areas of data quality and clinical performance in need of improvement. |
2. | In collaboration with Chief Quality Officer, the CommuniCare QI Committee, and the QA Supervisor, co-coordinates with current PCC all QI activities for Hansen Clinic. |
3. | Works with clinic staff to optimize use of the EHR to ensure data quality |
4. | Supports tracking and follow up of abnormal cervical cancer screening & abnormal breast cancer screening. |
5. | Provides trainings for medical assistants on preventative health care and quality improvement. |
6. | Supports Every Woman Counts (EWC) case management at Hansen Clinic. Ensures that cervical and breast cancer screening results (including data entry and patient follow) up are conducted in compliance with EWC guidelines. |
7. | Ensures EWC program applications are completed accurately at Hansen Clinic. |
8. | Supports Immunization Services Supervisor. Ensures compliance with VFC/VFA guidelines at Salud Clinic and supports QI efforts to increase vaccination rates. |
9. | Supports tracking of provider/clinic productivity data and attends Team Meetings. |
10. | Coordinates special projects as needed. |
11. | Works along side COVID PCCs to meet the quality improvement needs of the clinic. |
12. | Communicates effectively with patients, medical staff, and outside medical organizations to assist clients with scheduling appointments, obtaining medical records and problem solving as needed. |
