Overview

Plays a pivotal role in managing and overseeing the delivery of core services and supports for youth designed to address the mental, emotional, behavioral, and physical well-being of clients. Acts as a bridge between various service providers, ensuring that all services are integrated and aligned with the youth’s Needs and Services Plan. Ensures that youth receive comprehensive, individualized services that facilitate their successful treatment and transition back to community or family-based settings. We also provide free supervision towards licensure and collaboration with treatment teams including psychiatric interventions.

Job Duties and Responsibilities:

  • Creates a comprehensive client profile that informs the entire treatment team of essential information at the time of placement to ensure a smooth transition. This includes but is not limited to medication needs, safety concerns, triggers and trauma history, and the level of family engagement.
  • Assembles the Family Team by interviewing the client, the client’s family, identifying extended family supports, natural supports, community supports and formal supports.
  • Gathers information on the client’s mental health, social, and behavioral needs through the Child and Adolescent Needs and Strengths tool.
  • Assists the client and family in understanding and fully participating in the Child and Family Team process. Ensures that the client and the clients’ family are involved in all phases of determining the goals and needs.
  • Ensures that the legal and court requirements for clients under WIC Section 300 or 600 are met throughout their placement.
  • Provides telephone support and crisis de-escalation to client and family, 24 hours a day/7 days a week, including holidays, as assigned.

Required Qualifications:

  • Possess a Master’s Degree in Social Work, and experience working with clients in an employment or volunteer capacity. ACSW or LCSW preferred.
  • Working knowledge of computer programs including Outlook and Word. Trainable in use of Electronic Health Record.
  • Possess good social skills and demonstrate a high level of professionalism, common-sense and be able to exercise good judgment.
  • Must be able to pass a criminal background clearance and possess a valid CDL with a good driving record plus reliable transportation.

  • Up to 48 hours per year for Bereavement with death in immediate family
