THIS POSITION IS FULLY REMOTE BUT MUST BE LICESEND IN NY STATE

We are currently seeking A Remote Nurse Practitioner to provide Remote Telehealth to patients residing in their personal homes.

NP ROLE AND RESPONSIBILITIES:

· Provide primary and preventative care for All ages remotely.

· Perform patient histories and physical examinations.

· Evaluate findings and provide appropriate patient care

· Emphasize early detection of ailments, prevention, and all-around wellness.

· Conducting thorough annual assessments.

· Appropriately referring patients who have ongoing or acute medical needs.

· Manage Patients with any changes in condition.

· Prescribe appropriate medications, including controlled substances in accordance with NY Law.

· Order, review, and/or follow up with treatments and diagnostic testing.

· Communicate with the patients on a as needed basis to discuss changes in condition.

· Quality Improvement and Quality Assurance.

· Collaborate with primary care physician and medical personnel involved.

· Strong Interpersonal and communication skills with the ability to engage at all levels of the organization to promote a culture of patient safety

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS:

· Current, unrestricted NP license in the state of New York.

· Board Certification.

· DEA licensure.

· Clinical background in adult, family, and geriatric patients.

· Experience working in a Primary care and Urgent Care setting.

· Ability to work with an EMR. ECLINICAL

· Ability to manage patients with advanced illness and multiple chronic conditions.

· Ability to work within an interdisciplinary care team to coordinate patient care.
