Job Title: Community Health Worker (CHW)
Reports To: Program Director/Coordinator
Location: Omni Resource Services Public Health and Wellness Center, Louisville, KY
Job Summary:
Omni Resource Services is seeking a passionate and dedicated Community Health Worker (CHW) to join our team. The CHW will play a vital role in promoting the health and well-being of vulnerable populations by providing outreach, education, and navigation support. This position will focus on delivering innovative, barrier-free services, including harm reduction strategies, community-based care coordination, and addressing Social Determinants of Health (SDOH).
Key Responsibilities:
- Community Outreach and Engagement:
- Conduct outreach activities to connect underserved populations with Omni’s services, including harm reduction, substance use treatment, and healthcare coordination.
- Build and maintain relationships with community members and local organizations to enhance trust and collaboration.
- Education and Advocacy:
- Provide education on topics such as substance use, mental health, Medication-Assisted Treatment (MAT), sexual health, chronic diseases, and harm reduction strategies.
- Advocate for clients by addressing barriers to healthcare, housing, and other essential services.
- Client Navigation and Support:
- Assist clients in accessing healthcare services, transportation, social programs, and other resources.
- Conduct needs assessments to identify gaps in services and connect clients to appropriate programs.
- Provide follow-up support to ensure clients remain engaged in care plans.
- Harm Reduction Services:
- Distribute harm reduction supplies (e.g., naloxone, safe injection kits) and provide education on their use.
- Promote overdose prevention and provide support for individuals in crisis.
- Documentation and Reporting:
- Maintain accurate and timely records of client interactions and outreach activities.
- Assist in tracking program outcomes and preparing reports as required.
- Collaboration:
- Work closely with Omni’s interdisciplinary team, including Case Managers, Community Health Workers, and clinical staff (APRNs, RNs), to ensure seamless service delivery.
- Participate in team meetings, training sessions, and professional development opportunities.
Qualifications:
- Required:
- High school diploma or equivalent.
- Lived experience or demonstrated understanding of the challenges faced by marginalized populations.
- Strong interpersonal and communication skills, with the ability to build trust with clients and community members.
- Proficiency in Microsoft Office and basic data entry systems.
- Valid driver’s license and reliable transportation.
- Preferred:
- Certification as a Community Health Worker (CHW) or willingness to complete CHW training through Omni’s programs.
- Experience in harm reduction, public health, or social services.
- Knowledge of Louisville’s community resources and social service networks.
