Job Title: Community Health Worker (CHW)

Reports To: Program Director/Coordinator
Location: Omni Resource Services Public Health and Wellness Center, Louisville, KY

Job Summary:

Omni Resource Services is seeking a passionate and dedicated Community Health Worker (CHW) to join our team. The CHW will play a vital role in promoting the health and well-being of vulnerable populations by providing outreach, education, and navigation support. This position will focus on delivering innovative, barrier-free services, including harm reduction strategies, community-based care coordination, and addressing Social Determinants of Health (SDOH).

Key Responsibilities:

• Community Outreach and Engagement:
• Conduct outreach activities to connect underserved populations with Omni’s services, including harm reduction, substance use treatment, and healthcare coordination.
• Build and maintain relationships with community members and local organizations to enhance trust and collaboration.
• Education and Advocacy:
• Provide education on topics such as substance use, mental health, Medication-Assisted Treatment (MAT), sexual health, chronic diseases, and harm reduction strategies.
• Advocate for clients by addressing barriers to healthcare, housing, and other essential services.
• Client Navigation and Support:
• Assist clients in accessing healthcare services, transportation, social programs, and other resources.
• Conduct needs assessments to identify gaps in services and connect clients to appropriate programs.
• Provide follow-up support to ensure clients remain engaged in care plans.
• Harm Reduction Services:
• Distribute harm reduction supplies (e.g., naloxone, safe injection kits) and provide education on their use.
• Promote overdose prevention and provide support for individuals in crisis.
• Documentation and Reporting:
• Maintain accurate and timely records of client interactions and outreach activities.
• Assist in tracking program outcomes and preparing reports as required.
• Collaboration:
• Work closely with Omni’s interdisciplinary team, including Case Managers, Community Health Workers, and clinical staff (APRNs, RNs), to ensure seamless service delivery.
• Participate in team meetings, training sessions, and professional development opportunities.

Qualifications:

• Required:
• High school diploma or equivalent.
• Lived experience or demonstrated understanding of the challenges faced by marginalized populations.
• Strong interpersonal and communication skills, with the ability to build trust with clients and community members.
• Proficiency in Microsoft Office and basic data entry systems.
• Valid driver’s license and reliable transportation.
• Preferred:
• Certification as a Community Health Worker (CHW) or willingness to complete CHW training through Omni’s programs.
• Experience in harm reduction, public health, or social services.
• Knowledge of Louisville’s community resources and social service networks.
