Job Title: Initial Assessment Nurse / Admission Nurse – Adult Foster Care (AFC)Location: BrocktonJob Type: Full Time/Part TimJob Description:We are seeking a compassionate and detail-oriented Initial Assessment Nurse / Admission Nurse to conduct home visits and assess individuals for admission into our Adult Foster Care (AFC) program. The ideal candidate will have a strong clinical background and experience in home health, geriatric care, or community nursing.
Key Responsibilities:
- Conduct initial assessments of prospective AFC members in their homes.
- Evaluate medical and functional to determine eligibility.
- Develop care plans and collaborate with caregivers and healthcare teams.
- Educate families and members on program benefits, policies, and expectations.
- Ensure compliance with state regulations and AFC program guidelines.
- Maintain accurate and timely documentation of assessments and recommendations.
Qualifications:
- Active RN license in MA.
- Minimum 1 Year of expereince in Initial Assessments.
- Experience in home health, case management, or geriatric care preferred.
- Strong assessment, communication, and documentation skills.
- Ability to travel locally for home visits.
- Knowledge of Medicaid, AFC regulations, and community resources is a plus.

Pay: $65.00 - $80.00 per hour
