Clinical Team Liaison

This position will be based out of Spokane, but mostly work from home.
Foster the growing On it RN footprint in Washington state (and surrounding areas) by empowering and championing for the nursing team to excel to the peak of their professional abilities. Serving as a clinical liaison for the nursing team, this position will provide support to and remove barriers from the nurses by coordinating working, collaboration, and proactive problem-solving.
Accurate Caregiver Addition to Nurse Visit Notes:
- Metric: Percentage of caregiver information accurately recorded.
- Target: Maintain a 100% or higher accuracy rate in caregiver information addition.
Completion of Nursing Task Sheets:
- Metric: Percentage of nursing task sheets completed within 24 hours of receiving a new resident or task.
- Target: Ensure at least 95% of nursing task sheets are completed within the specified time frame.
Verification of Physician Orders:
- Metric: Percentage of physician orders verified before the nurse completes 90-day assessments.
- Target: Achieve a 95% verification rate of physician orders before the nurse conducts 90-day assessments.
Follow-up on Missing or Unverified Orders:
- Metric: Average time taken to follow up on missing or orders needing verification.
- Target: Respond to and follow up on missing or unverified orders within 24 hours of identification.
Data Entry for RapidAssess:
- Metric: Percentage of completed demographic and data entry information for RapidAssess relative to the total number of assessments.
- Target: Maintain a 95% or higher completion rate for RapidAssess data entry before RN completes assessment phone calls.
These metrics provide clear targets and measurable outcomes for the clinical team liaison, ensuring a focus on accuracy, timeliness, and completeness in their various responsibilities. Regular monitoring of these metrics will help in identifying areas for improvement and ensuring consistent high-quality performance.
