Associate Clinical Social Worker (ACSW) - Street MedicineThe Street Medicine ACSW provides behavioral health services to individuals experiencing homelessness in their own environments, such as encampments, parks, and other non-traditional settings. The ACSW will offer trauma-informed counseling, support, and referrals to address the mental health and social needs of this vulnerable population. Working alongside a multidisciplinary team, the ACSW will help bridge gaps in care and reduce barriers by bringing services directly to those who may not access conventional healthcare systems.This role is on-site in Midtown Sacramento, CA (95811). Essential FunctionsProvide individual counseling using strength-based and trauma-informed approaches, meeting patients in non-traditional settings such as streets, shelters, and encampments.Conduct thorough behavioral health evaluations, developing personalized treatment plans and setting therapeutic goals in collaboration with patients.Offer ongoing support and follow-up to evaluate patient progress and modify treatment plans as needed, ensuring continuity of care.Administer therapeutic interventions including cognitive-behavioral therapy, behavior modification, and stress management techniques to address mental health and substance use issues.Collaborate with medical providers, nurses, case managers, and other members of the Street Medicine team to coordinate care and support the patient's overall health and well-being.Refer patients to additional mental health services, including psychiatric care and psychotropic medication management, when appropriate.Provide education and counseling on harm reduction, substance use, trauma recovery, and mental health strategies tailored to individuals experiencing homelessness.Participate in integrated systems of care, including staff meetings, case reviews, and supervision, to ensure a collaborative approach to patient care.Maintain detailed, systematic documentation of patient encounters and treatment plans in the electronic medical record (EMR), ensuring timely and accurate records for clinical and billing purposes.Additional DutiesParticipate in street rounds with the outreach team and attend case conferences and team meetings as required.Assist with coordinating care and referrals with external agencies, including shelters, harm reduction programs, and mental health services.Maintain professional development and participate in continuous learning opportunities related to street medicine, behavioral health, and trauma-informed care.Report suspected cases of child, elder, dependent adult, or domestic abuse in compliance with applicable legal mandates (Penal Code, Section 11166, Section 15630, and Section 1160).Perform other duties as assigned by Street Medicine leadership.Travel ResponsibilitiesPrimarily local with occasional travel to community sites and partner organizationsTraveling to encampments, shelters, and other sites to meet the needs of the unhoused individuals.Candidate should possess reliable transportation, valid driver's license, and car insurance.Minimum RequirementsMaster's degree in social work required.ASW in good standing with the state of California.Experience providing therapy and counseling services, preferably with vulnerable or unhoused populations.Strong interpersonal and communication skills, with the ability to build rapport and trust with patients from diverse backgrounds.Ability to work effectively within a multidisciplinary team, adapting to the unique challenges of providing care in non-traditional settings.Commitment to person-centered, trauma-informed care and a willingness to work with marginalized populations in a street medicine environment.Active CPR and Basic Life Support (BLS) certification.Preferred BackgroundExperience in any of the following areas : Substance Use DisorderTrauma RecoveryLGBTQIA and Gender-Affirming HealthYouth Mental HealthHarm Reduction ApproachesFamiliarity with restorative practices and integrated care models.Passion for serving vulnerable populations and a commitment to improving health equity.One Community Health serves a widely diverse patient population with a rich blend of races, ethnicities, cultures, ages, religions, disabilities, sexual orientations, gender identities, and socioeconomic backgrounds.
