Linkage and Retention Specialist
The Linkage and Retention Specialist is a critical role in HIV care, focusing on connecting patients to appropriate treatment and ensuring their continued engagement in care programs. Key responsibilities include facilitating patient integration into healthcare services and engaging in proactive outreach to re-engage and retain patients. This position requires strong communication, empathy, and collaborative skills, essential for effective patient care and community engagement in the field of HIV treatment.
Essential Functions :
- Facilitate the linkage of newly diagnosed, re-engaged, and transfer patients living with HIV into care and treatment programs from community-based organizations, health care systems, self-referrals and internally, ensuring their smooth transition and understanding of available services.
- Coordinate patient care by working as an integral part of the care team to retain patients in the healthcare system, working with case management and clinicians to implement personalized care plans, provide education and monitoring compliance to medical appointments and lab work.
- Conduct proactive outreach through field visits, letters, and phone calls to engage patients who have missed appointments or fallen out of care, identifying and addressing the reasons for their disengagement.
- Provide acute support to patients through the HIV Program Line, instant messaging, and direct communication, acting as the first point of contact for patients, clinicians, and staff within the agency for HIV-related inquiries.
- Implement Quality Improvement (QI) initiatives aimed at identifying and overcoming challenges and barriers faced by patients, enhancing the overall effectiveness and accessibility of HIV treatment and care services.
