What are the responsibilities and job description for the Contract Therapy Coordinator position at One Home Health Agency, Ltd.?
We are looking for a Contract Therapy Coordinator to join our team who will manage, supervise, coordinate, evaluate, and develop client care teams to ensure quality care delivery and appropriate case management by all third-party therapy providers the Agency has contracted within scope of services and policies; state, federal, and local laws.
Qualifications:
-Current license to practice as a Physical Therapist or Physical Therapist Assistant
-Minimum of one year of home health experience
-Complies with accepted professional standards and practice
-Excellent verbal and written communication skills and strong interpersonal skills
-Knowledge of home care federal and state regulations
-Knowledge of reimbursement sources and documentation requirements within home health care
-Possess basic computer skills
Responsibilities:
-Manages and directs contract therapy staff of contracts that the Agency utilizes to care for patients
-Is available at all times during business hours and as needed to provide support and assure quality care delivery to home care clients
-Monitors all patients referred to contract companies, approves care plan, monitors utilization and any changes to frequency and/or orders, and coordinates appropriate discharge dates
-Maintains communication with directors/managers and field staff of contract companies
-Reviews and approves contract therapy invoices for payment to ensure all visits were performed by Agency's standards
-Provides support and direction to contact staff
