Job Description:
We are seeking a dedicated and compassionate Intact Case Manager to join our team in Effingham, IL. The primary responsibility of the Intact Case Manager is to provide comprehensive case management services to families involved in the child welfare system. This includes conducting assessments, developing service plans, coordinating services, and monitoring progress towards goals.
Responsibilities:
- Conduct initial assessments to determine family needs and strengths
- Develop individualized service plans in collaboration with families
- Coordinate and monitor services provided to families, including mental health, substance abuse, and parenting support
- Provide crisis intervention and support to families in times of need
- Maintain accurate and up-to-date case records
- Collaborate with community partners and agencies to ensure comprehensive care for families
Qualifications:
- Bachelor's degree in social work, psychology, or related field
- Previous experience working with children and families in a social service setting
- Knowledge of child welfare laws and regulations
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation
