What are the responsibilities and job description for the Therapeutic Foster Care Case Manager - $5,000 Sign OnBonus position at One Hope United?
Under the assigned Therapeutic Foster Care Supervisor, provide case management services for a caseload of ten treatment foster care children and their families. Will ensure that the child’s needs for safety, permanency, wellbeing and placement stability are met within the context of an evidence-based model of treatment foster care using a trauma informed approach.
Essential Duties and Responsibilities
- Meet individually with the youth at least three times per month, with at least two in-person visits occurring in the Therapeutic Foster Home.
- Meet with the Therapeutic Foster Parent(s) at least once per month.
- Coordinate all treatment services to ensure developmentally appropriate therapeutic services are provided to all youth as recommended by the assessment process and included in the treatment plan to meet individualized goals.
- Create Service Plans based on Integrated Assessments and needs of the youth/family identified in the IM CANS, including actionable steps to meet desired permanency outcomes.
- Document all case-related interactions and events with youth and service providers, as prescribed by DCFS policy, and maintain client files by submitting and filing documentation within prescribed timeframes.
- Facilitate transitions into, out of, and between homes and/or treatment facilities.
- Assist in placement stabilization by supporting Treatment Foster Parents through increased contact, as needed.
- Ensure required and recommended visitations occur, e.g. sibling, parent-child.
- Attend all court hearings, Administrative Case Reviews, school staffings, etc. and interact professionally with outside providers and case-related personnel.
- Provide crisis intervention as needed with 24-hour access, as needed.
- Maintain required contact with and ensure recommended support for biological families.
- Work independently on an individual basis with the youth surrounding their treatment and service needs.
- Communicate pertinent events/aspects of a youth’s case with all team members on an ongoing basis
- Organize, manage and lead document Child and Family Team Meetings per DCFS policy.
- Assists in family reunification and stabilization efforts with the youth’s primary family or other permanent living arrangement consistent with the youth’s case plan.
- BSW/BA degree required
Experience
- 2 years of direct child welfare experience required
Licensure
- CWEL, CANS, CERAP, placement specialty eligible
Required Skills
- Availability to participate in evidence-based model training, coaching and fidelity to certification once hired required.
- Ability to work independently.
- Ability to produce results while working in a hybrid location.
- Willingness to work flexible and non-traditional hours, including evening and weekends, as needed.
- Demonstrate the ability to communicate in a clear, comprehensible manner, both verbally and in writing.
- Demonstrate the ability to motivate children, their families and foster parents to participate in a highly detailed treatment model.
- Valid driver’s license, in good standing for the state of residency required.
- Reliable personal transportation
