Registered Nurse Case Manager

Responsibilities
- Conduct comprehensive assessments of patients' medical conditions and develop individualized care plans
- Collaborate with physicians, specialists, and other healthcare professionals to coordinate patient care
- Monitor patients' progress and make necessary adjustments to treatment plans
- Provide education and support to patients and their families regarding their healthcare needs
- Ensure accurate and timely documentation of patient information in medical records
- Advocate for patients' rights and ensure they receive appropriate healthcare services
- Collaborate with insurance providers to ensure proper utilization of healthcare resources
- Participate in case conferences and multidisciplinary team meetings to discuss patient progress and treatment options
- Valid Registered Nurse (RN) license in [state]
