Responsibilities
- Assist individuals with daily living activities to promote independence.
- Monitor and document patient behavior, health status, and progress towards goals.
- Implement personalized care plans in collaboration with families and care teams.
- Provide emotional support and encouragement to individuals and their families.
- Facilitate social interactions and community engagement activities.
- Ensure a safe, clean, and supportive environment for all individuals.
- Advocate for the needs of individuals within the healthcare system.
Qualifications
- Experience in direct support or caregiving roles preferred.
- Strong observational skills to monitor patient behavior effectively.
- Ability to work collaboratively with families and interdisciplinary teams.
- Compassionate approach towards individuals with disabilities.
- Excellent communication skills for effective interaction with clients and team members.
