As an LPN Intake Coordinator at OneWell Health Care, you will play a vital role in assessing the needs of clients and coordinating their care plans. This position is essential in ensuring that we deliver high-quality, person-centered healthcare services that promote healthy living and well-being.
Responsibilities
- Conduct comprehensive patient assessments to determine individual needs and preferences.
- Coordinate with families and care teams to develop tailored care plans.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
- Utilize electronic health record systems such as Epic or eClinicalWorks for documentation and communication.
- Facilitate smooth transitions into care by providing necessary information to clients and families.
- Collaborate with healthcare professionals to ensure comprehensive support for clients.
Requirements
- Experience in patient assessment or intake coordination in a healthcare setting such as home health and or hospice is preferred.
- Strong understanding of HIPAA regulations and patient confidentiality requirements.
- Excellent communication skills for effective interaction with clients, families, and team members.
- Ability to work independently and as part of a team.
- Compassionate approach towards individuals with developmental disabilities.
