The Clinical Coordinator ensures smooth coordination, documentation, and compliance for mental and behavioral health services. Responsibilities include submitting Service Registration Forms (SRFs), managing assessments, coordinating care, and ensuring compliance with Medicaid and ASAM guidelines.
Key Responsibilities:
- Service Registration & Documentation: Submit SRFs, ASAM documents, and initial assessments for PHP and other programs.
- Care Coordination: Communicate with clinical staff, providers, and agencies to ensure continuity of care.
- Assessments & Reports: Complete quarterly and SRA assessments for MHSB, IIH, and CS programs.
- ASAM & PHP Support: Ensure proper documentation and service placement for PHP level 2.5 clients.
- Compliance & Quality Assurance: Maintain Medicaid, DMAS, and agency documentation standards.
- Administrative Support: Assist clinical leadership with reporting, tracking, and case coordination.
- Licensure: LPC, LCSW, LMFT, or LMHP-R/LMHP-S (required).
- Experience: 2 years in mental health administration; knowledge of MHSB, IIH, CS, ASAM, and PHP 2.5.
- Skills: Strong organization, communication, and EHR proficiency.
